Index-Patient

Als Indexpatient (auch „Patient Null“, engl. patient zero) bezeichnet man in der Epidemiologie jene Person, von der aus die Ausbreitung einer Erkrankung, meist einer Infektion, ihren mutmaßlichen Ursprung genommen hat.

Der Indexpatient im Blutspendewesen ist beispielsweise jener Spender, bei dem durch eine infektiöse Blutspende Erreger auf einen oder mehrere Empfänger von Blutprodukten übertragen wurde. Der infizierte Empfänger und der zu ermittelnde Indexpatient sind die wichtigsten Ausgangspunkte für ein sogenanntes Look-Back-Verfahren, bei dem alle Spender für einen bestimmten Empfänger sowie alle Empfänger von Spenden des Indexpatienten ermittelt werden müssen.

Bei Verdacht einer nosokomialen Infektion oder einem Arbeitsunfall mit potentiell infektiösem Blut u.ä (z. B. Nadelstichverletzung), wird jener Patient, von dem das Blut oder die Nadel stammt, als Indexpatient bezeichnet.

Die Rekonstruktion der Ausbreitung von Epidemien konzentriert sich wesentlich auf die Suche nach dem Indexpatienten, um so die weiteren Kontaktpersonen und die Ausbreitungsrouten des Erregers zu erfahren. Diese Suche nach dem Indexpatienten geschah beispielsweise bei der Untersuchung der SARS-Epidemie und bei den ersten bekannt gewordenen AIDS-Erkrankungen; der bekannteste Indexpatient ist im Zusammenhang mit HIV der sogenannte Patient Zero Gaëtan Dugas, von dem man zeitweise glaubte, dass man die Ausbreitung der Infektion in den USA auf ihn zurückverfolgen könne.

In der Systemischen Therapie wird ein Mitglied einer Gruppe, das mit seinen Symptomen einengende Wirklichkeitskonstruktionen und leiderzeugende Interaktionsmuster innerhalb der Gruppe anzeigt, ebenfalls als Indexpatient bezeichnet.[1]
