Die perkutane transhepatische Drainage (PTD) ist ein röntgenologisches Verfahren der direkten perkutanen Draineinlage in die Gallenwege nach erfolgreicher perkutaner transhepatische Cholangiographie (PTC), bei welcher mit Hilfe einer dünnen Hohlnadel unter Sedierung und Lokalanästhesie perkutan durch Punktion der Leber in das Gallenwegsystem Röntgenkontrastmittel eingebracht wird. Das Verfahren wird alternativ zur ERCP angewandt.

Durch die PTD kann gestaute Gallenflüssigkeit nach außen über eine Drainage abgeleitet werden. Durch mehrschrittiges Aufdehnen des Zugangsweges kann eine (Cholangioskopie), d.h. eine direkte Endoskopie der Gallenwege erfolgen.

Wenn eine Passage einer Gallengangsstenose gelingt, kann eine innere Ableitung der Gallenflüssigkeit in den Dünndarm erfolgen. In einem solchen Fall ist es möglich, perkutan einen Stent zu legen, der den Galleabfluß aus den Gallenwegen in den Dünndarm langfristig wiederherstellt. (s. Abb.)

Bei Gallengangssteinen ist es möglich per Cholangioskopie eine Steinzertrümmerung (z.B. elektrohydraulisch) durchzuführen.
