Thienamycin

Carbapeneme, ursprünglich als Thienamycine bezeichnet, sind β-Lactam-Antibiotika mit breitem Wirkungspektrum. Vertreter sind Imipenem, Ertapenem, Meropenem und Doripenem.

Imipenem ist stark nephrotoxisch (nierenschädlich). Zur Nierenprotektion und Verlängerung der Halbwertszeit wird das Antibiotikum mit dem Dehydropeptidasehemmer Cilastatin kombiniert (im Handel etwa als Zienam®). So wird die renale Hydrolysierung des Medikaments hinausgezögert. Bei den anderen Carbapenemen ist diese Kombination nicht erforderlich.

Carbapeneme werden zur Therapie von schweren nosokomialen Infektionen, wie etwa durch Pseudomonas aeruginosa hervorgerufen, eingesetzt.
