Bronchodilator

Ein Bronchospasmolytikum ist ein Arzneimittel, das den Tonus der Bronchialmuskulatur senkt und damit eine Weitung der Bronchien bewirkt. Bronchospasmolytika werden typischerweise zur Behandlung des Asthma bronchiale eingesetzt.

Wirkstoffgruppen

Pharmakologisch werden drei Wirkstoffgruppen von Bronchospasmolytika unterschieden:

β-Adrenozeptoragonisten

Die Stimulation des β2-Adrenozeptors durch β2-Sympathomimetika führt zur Entspannung des Bronchialmuskulatur und löst einen Bronchospasmus. Vertreter dieser Substanzgruppe sind beispielsweise Salbutamol, Salmeterol, Terbutalin, Clenbuterol oder Formoterol.

Theophyllin

Theophyllin ist ein Xanthinderivat, dessen Wirkmechanismus noch nicht vollständig geklärt ist. Zum einen hemmt es in hohen Dosen das Enzym Phosphodiesterase, wodurch die Konzentration des second Messenger cAMP in der bronchialen Muskelzelle erhöht wird. Zum anderen blockiert Theophyllin die Adenosinrezeptoren, die sonst eine Verengung der Bronchien und die Ausschüttung von Histamin bewirken.

Muskarinrezeptorantagonisten

Parasympatholytika wie beispielsweise Ipratropiumbromid verhindern die Bindung des Neurotransmitters Acetylcholin an den Muskarinischen Acetylcholinrezeptor.
