Ein Auftragsforschungsinstitut (auch Auftragsinstitut bzw. CRO, Contract Research Organisation oder Clinical Research Organization) dient der Planung und Durchführung klinischer Studien. Es handelt sich hierbei um Dienstleistungsunternehmen für die pharmazeutische und Medizinprodukte produzierende Industrie, welche streng genommen die Forschung und Entwicklung von Arzneimitteln bzw. Medizinprodukten unterstützen. Es gibt weltweit über 1.000 verschiedene Unternehmen (der Marktführer hat z. B. 17.000 Mitarbeiter weltweit). Alleine in Deutschland existierten 2005 ca. 60–100 verschiedene Unternehmen dieser Kategorie. Eine vollständige Liste existiert nicht. Der „Bundesverband der Pharmazeutischen Industrie“ (BPI) führt jedoch eine Liste, in der ca. 60 der in Deutschland aktiven Auftragsforschungsinstitute aufgeführt werden. Einige der etabliertesten Auftragsforschungsinstitute haben sich in Deutschland zum „Bundesverband der medizinischen Auftragsinstitute“ (BVMA) zusammengeschlossen. Auf internationaler Ebene haben sich CROs zur ACRO (Association of Clinical Research Organizations) mit Sitz in Washington, DC (USA) zusammengeschlossen. Laut der Zeitschrift Pharmaceutical R&D Outsourcing Strategies lag der Marktanteil der 10 größten und weltweit operierenden CROs 2001 bei 38 %: Quintiles 10 %, Covance 9 %, PPD 5 %, Parexel 4 %, MDS 3 %, Inveresk (jetzt: Charles River Labs) 2 %, Kendle 2 %, ICON 1 %, OminCare 1 % und PRA 1 %.
