Der Coombs-Test (selten auch Race-Coombs-Test) ist ein nach dem Cambridger Pathologen Robert Royston Amos Coombs benannter Antiglobulintest. Mit ihm werden inkomplette Antikörper (IgG) gegen rote Blutkörperchen (Erythrozyten) nachgewiesen.

  • Der direkte Coombs-Test dient dem Nachweis inkompletter Antikörper, die an Erythrozyten gebunden sind.
  • Der indirekte Coombs-Test weist nicht-gebundene, im Serum zirkulierende, inkomplette Antikörper nach. Der Test hat Bedeutung für den Nachweis inkompletter Antikörper im Serum der Mutter bei Rhesusinkompatibilität.

Ein positiver Coombs-Test kann z. B. bei folgenden Erkrankungen auftreten: Autoimmunerkrankungen (rheumatische Erkrankungen), Infektionen, lymphatischen Leukämien, Medikamentennebenwirkung. Der Coombs-Test kann allerdings auch ohne erkennbare Ursache (idiopathisch) positiv sein. Ist der Coombs-Test negativ, kann es sich um hereditäre Anämien wie die Thalassämie handeln.
