High 5s

Das Projekt High 5s ist eine internationale Kooperation zur weltweiten Implementierung von Maßnahmen zur Verbesserung der Patientensicherheit, die 2006 von der WHO ins Leben gerufen wurde. Der Name High 5s bezieht sich auf die ursprünglichen Projektziele, nach denen im Lauf von 5 Jahren die Häufigkeit von 5 bedeutsamen Patientensicherheitsproblemen in 5 Ländern signifikant reduziert werden sollte.

Organisation

Folgende Länder waren die ersten Projektbeteiligten: Australien, Deutschland, Kanada, Neuseeland, die Niederlande, das Vereinigte Königreich und die USA. Frankreich, Saudi Arabien und Singapur schlossen sich der Initiative später an. In all diesen Staaten wurden als nationale Koordiationszentren sogenannte Lead Technical Agencies (LTA) eingerichtet. In Deutschland sind dies – mit Unterstützung des Bundesministeriums für Gesundheit das Ärztliche Zentrum für Qualität in der Medizin und das Aktionsbündnis Patientensicherheit. Die Gesamtkoordination erfolgt durch Joint Commission und Joint Commission International als WHO Collaborating Centre for Patient Safety.

Ziele

Ziele von High 5s sind die Implementierung und Evaluation standardisierter Maßnahmen zur Verbesserung der Patientensicherheit.

Aktivitäten

Zu den wesentlichen Aktivitäten des Projektes gehören die Entwicklung, Implementierung und Analyse Problem-spezifischer Standard-Protokolle (SOPs) sowie entsprechender Qualitätsindikatoren – unter anderem zur Vermeidung von Behandlungsfehlern und zur Optimierung folgender Probleme in der Patientenversorgung:

  1. Sichere Pharmakotherapie, insbesondere bei Einweisung ins und Entlassung aus dem Krankenhaus,
  2. Sicherer Umgang mit hochkonzentrierten Injektions-Lösungen,
  3. Rechts-Links-Verwechslungen bei Operationen,
  4. Kommunikationsfehler zwischen Behandelnden,
  5. Iatrogene Infektionen.
