Die Familiäre adenomatöse Polyposis (syn. FAP, Polyposis coli) ist eine autosomal-dominant vererbbare Erkrankung. Diese führt zu Polypen im Dickdarm, die unbehandelt entarten und zu Darmkrebs führen (so genannte obligate Präkanzerose).

Kolonkarzinome (Dickdarmkrebs) entstehen etwa zu 1 % bei FAP, wohingegen etwa 10 % der Fälle auf einem HNPCC beruhen sollen, und die große Mehrzahl, 89 %, sogenannte sporadische Karzinome sind, die ohne zugrundeliegenden hereditären (erblichen) Gendefekt entstehen.

Ursache

Ursache der Erkrankung ist eine Keimbahnmutation des APC-Gens (Adenomatous Polyposis of the Colon). Dieses auf Chromosom 5 5q21 liegende Tumorsuppressorgen ist dabei inaktiviert. Beim Gesunden bindet APC-Protein an β-Catenin und vermittelt über Ubiquitinylierung dessen Abbau im Proteasom. Kann β-Catenin nicht mehr ubiquitinyliert werden, enthalten die Zellen dadurch große Mengen an β-Catenin. Dieses transloziert in den Zellkern und wirkt dort gemeinsam mit anderen Faktoren als Transkriptionsfaktoren. Dadurch wird das Zellwachstum gefördert und Telomerase gebildet, die Zellen immortaliert. Es kommt zu einem beschleunigten Abblauf der Adenom-Karzinom-Sequenz.

Therapie und Verlauf

Bei Trägern der Erkrankung entstehen ab dem 10. Lebensjahr Polypen im Dickdarm. Da die Wahrscheinlichkeit fast 100 % beträgt, dass einer oder mehrere dieser hundert bis tausenden Polypen mit der Zeit zu einem Kolonkarzinom entarten, wird wenn möglich vor dem 20. Lebensjahr der Dickdarm einschließlich des Mastdarms operativ entfernt (Proktokolektomie).

In einigen Fällen treten allerdings Dünndarmkarzinome und progressiv wachsende Desmoidtumore auf.
