Cephalosporine sind eine Gruppe von Breitband-Antibiotika für den medizinischen Einsatz. Wie auch die Penicilline gehören sie der Gruppe der β-Lactam-Antibiotika an. Sie wirken bakterizid auf proliferierende Bakterien, d. h. sie töten sich teilende Bakterien, indem sie ihre Zellwandsynthese stören.

Natürlicherweise kommen Cephalosporine im Schimmelpilz Acremonium chrysogenum (ehemals Cephalosporium acremonium) als Cephalosporin-C vor. Cephalosporin-C wurde 1945 erstmals von Giuseppe Brotzu isoliert, die chemische Struktur wurde 1953 aufgeklärt. Es dient bis heute als Grundsubstanz der Herstellung; Cephalosporine sind somit halbsynthetische Arzneistoffe.

Einteilung

Neben den „klassischen“ Cephalosporinen gibt es eine Reihe von Abwandlungen, die als Cephalosporine der zweiten und dritten Generation bezeichnet werden. Der Vorteil der zweiten Generation ist eine bessere Resistenz gegen β-Lactamase, einem Enzym, mit dem sich einige Bakterien, insbesondere gramnegative Stäbchenbakterien, gegen β-Lactam-Antibiotika (s. o.) wehren können. Die dritte Generation hat ein breiteres Wirkungsspektrum, erfasst also weitere Bakterien. Mindestens 18 Substanzen waren in den 1990er Jahren auf dem deutschen Markt zugelassen; nach einer Konsolidierungsphase enthält die Rote Liste jetzt noch neun Cephalosporine mit guter Verträglichkeit und Wirksamkeit. Bezeichnungen wie „4.“ und „5. Generation“ entstammen Marketingaussagen der Hersteller.
