α1-Antitrypsin (AAT), besser α1-Protease Inhibitor (α1PI), ist ein Akute-Phase-Protein, das Körpergewebe vor an Entzündungsprozessen beteiligten Enzymen schützt. Es wirkt, indem es vor allem PMN-Elastase, aber auch Trypsin und andere Serinproteasen hemmt. Fällt diese Hemmung durch einen Mangel oder ein in der Funktion eingeschränktes α1IP aus, greift die Elaste beispielsweise das Bindegewebe der Lunge an und es kann zu schweren Komplikationen kommen,

Die normale Konzentration des α1-Protease-Inhibitor im Blutplasma beträgt 8,3-19,9 mg/l. Im Zuge entzündlicher Prozesse erhöht sich der Wert. Im Vergleich zum C-reaktive Protein ist die Aktivitätsmessung von α1PI ein besserer Marker, um den Verlauf einer Entzüdung zu beobachten. Bei Verdacht auf einen Mangel an Alpha-1-Antitrypsin ist eine gleichzeitige Bestimmung des C-reaktiven Proteins sinnvoll, da eine Entzündung den Mangel verdecken kann.
